Stop in the Child Support Office to complete an Enrollment Form

Documentation that you will need:

Birth Certificate for each child

Social Security Card for each child

Applicant’s Driver’s License or State Issued Photo I.D.

Copies of any Court documents regarding divorce, paternity, guardianship, child support
